Registration Forms
Registration forms for Medford Sports Injury patients are available to be printed and completed prior to your first scheduled appointment. The forms that are included in this registration file allow us to learn about your condition and how it is affecting your daily life, understand your overall health, and to get an idea of what sort of rehabilitation will best suit your body's needs.

We at Medford Sports Injury realize that filling out paperwork is time consuming. However, we still need to have these forms filled out before you see our physical therapist.

The following forms are included:

 - Patient Registration
All patients must complete this form. Please bring your insurance card(s) with you to your first appointment, as we will need photocopies of them for our records.

 - Patient Health History
This form provides us with medical information that may affect your condition as well as treatment.


To view the above registration forms you must have Adobe Acrobat Reader. If you do not have Adobe Acrobat Reader, click on the icon below to download the current version.

To download the registration forms, right click on one of the forms below, then left click on "save target as" or "save as". This will save the form onto your computer, which you can open and print with Acrobat Reader after download is finished. Please be patient. You will need both forms.
