Registration forms for Medford Sports Injury patients are available to be printed and completed prior to your first scheduled appointment. The forms that are included in this registration file allow us to learn about your condition and how it is affecting your daily life, understand your overall health, and to get an idea of what sort of rehabilitation will best suit your body's needs.

We at Medford Sports Injury realize that filling out paperwork is time consuming. However, we still need to have these forms filled out before you see our physical therapist.

The following forms are included:

 - Patient Information & Health History
All patients must complete this form. Please bring your insurance card(s) and photo ID with you to your first appointment, as we will need them for our records.

- Payment Agreement & Assignment of Benefits

- Notice of Privacy Practices & Acknowledgement of Receipt
